Not all dental implants go into the bone the same way. Here's the real difference between endosteal and subperiosteal implants and who needs which.

What Is an Endosteal Implant?

Endosteal implants are the standard, most commonly used type of dental implant, shaped like a small screw and placed directly into the jawbone. If you picture a typical dental implant, this is almost certainly what you're imagining. Once placed, the surrounding bone gradually fuses to the implant surface through a process called osseointegration, creating a stable, long-term anchor for a crown, bridge, or denture.

What Is a Subperiosteal Implant?

Rather than going into the bone, a subperiosteal implant uses a custom-fitted metal frame that rests on top of the jawbone, underneath the gum tissue, with small posts extending up through the gum to support the replacement teeth. This approach doesn't require the same bone depth an endosteal implant needs, which is why it's historically been used for patients with significant bone loss who aren't candidates for a standard implant and either can't or don't want to undergo bone grafting first.

Why Does This Distinction Matter?

The choice between these two approaches essentially comes down to how much healthy jawbone you have available. Endosteal implants are generally preferred whenever there's adequate bone, since they've been more extensively studied, have a longer track record, and generally integrate more predictably over the long term. Subperiosteal implants are a more specialized option, generally reserved for cases where bone volume genuinely doesn't support a standard implant and grafting isn't a preferred path forward.

Which One Is More Common Today?

Endosteal implants are significantly more common in current dental practice. Advances in bone grafting techniques have also made it possible for many patients who once might have needed a subperiosteal approach to instead undergo grafting and receive a standard endosteal implant, which is part of why subperiosteal implants are used less frequently now than in past decades.

How Do the Procedures Differ?

Placing an endosteal implant involves a relatively standardized surgical process — creating a precise channel in the bone and inserting the implant post. A subperiosteal implant requires a more customized process, since the frame needs to be precisely fitted to the specific shape of your jawbone, sometimes involving a preliminary scan or impression to fabricate the frame before a second surgical visit to place it.

Recovery Differences

Recovery timelines for both types involve initial healing of the surgical site, generally similar in the first week or two. The deeper, long-term stability process differs somewhat, since an endosteal implant's success depends on bone fusing directly to the implant surface, while a subperiosteal implant's stability depends more on how well the frame fits and how the surrounding tissue heals around it.

Which Is Right for You?

This isn't a choice you make based on preference alone — it depends on your specific bone structure, evaluated through imaging by your dentist or oral surgeon. If you have adequate bone, an endosteal implant is almost always the recommended path. If bone loss is significant and grafting isn't the preferred approach for your situation, a subperiosteal implant or another alternative may be discussed instead.
