Internal resorption often causes no symptoms until it's advanced. Here's what it actually is, why it happens, and how it's typically treated.
What Is Internal Tooth Resorption?
Internal resorption is a condition where cells inside the tooth's pulp chamber begin breaking down the tooth's own internal structure from the inside out, gradually eating away at the dentin layer that normally surrounds the pulp.
Why Does This Happen?
The exact trigger isn't always clear, but it's generally associated with some form of chronic inflammation of the pulp tissue, sometimes following trauma to the tooth, or in some cases, without an obviously identifiable cause at all.
Why Doesn't It Cause Symptoms Early On?
Because the process happens inside the tooth, away from the nerve endings in a way that doesn't typically trigger noticeable pain in its earlier stages, many cases progress silently until either the tooth's structure becomes visibly affected or the condition is caught incidentally on an X-ray taken during a routine exam.
How Is Internal Resorption Diagnosed?
This condition is almost always identified through dental X-rays, which reveal a distinctive pattern of internal structure loss within the tooth, sometimes appearing as a rounded, hollowed-out area inside the tooth that's visible on imaging even without external symptoms.
What Happens If Internal Resorption Isn't Treated?
Left untreated, the process can continue progressing, eventually weakening the tooth structure enough to risk fracture, or in more advanced cases, resorption can progress to the point where it perforates through to the outside of the tooth, creating a connection between the pulp and the surrounding gum tissue.
How Is Internal Resorption Treated?
Root canal treatment is the standard approach, removing the affected pulp tissue that's driving the resorption process and sealing the canal to stop further progression, similar in concept to a standard root canal but specifically addressing this particular underlying cause.
Can Internal Resorption Be Reversed?
The tooth structure already lost to resorption doesn't regenerate, but appropriate root canal treatment can successfully stop the ongoing process from continuing to progress further, preserving the remaining tooth structure.
Is Internal Resorption Common?
This is considered a relatively uncommon dental finding compared to more typical issues like cavities or standard pulp infections, which is part of why it's not a condition most people are familiar with until they're personally told they have it.
Can a Tooth With Internal Resorption Be Saved?
In many cases, yes, particularly if caught before significant structural weakening or perforation has occurred, root canal treatment can successfully stop the process and preserve the tooth for continued function long-term.
What If the Resorption Has Already Weakened the Tooth Significantly?
In more advanced cases where the tooth structure has been substantially compromised, extraction may ultimately be necessary if the remaining tooth can't reliably support restoration and function, in which case a replacement option like an implant or bridge would be discussed.
